Medicaid / Katie Beckett

The Katie Beckett Program is a special eligibility process that allows certain children who are residents of Idaho with long-term disabilities or complex medical needs, living at home with their families, to obtain an Idaho Medicaid card.

Children who are not eligible for other Medicaid programs because the income or assets of their parents are too high, may be eligible for Medicaid through the Katie Beckett Program. However, they must meet all the eligibility criteria.

The following forms are included on the web site and must be completed and returned to the local Self-Reliance office:

Medicaid Application for child with a disability (Katie Beckett)
General Information for Katie Beckett Application
Authorization for Source to Release Information for Katie Beckett Application.
Physician’s Medical Care Evaluation for Alternative Care programs.
Child Information Form (an instruction sheet is included)

After you complete and return the required forms to your local Self Reliance office, you’ll have a required interview with a Self-Reliance caseworker.
If you have any questions or concerns about the medical information needed, please contact Regional Medicaid services at 208-334-0940.
